Attachments to an Adobe Pdf and signature

I am sending a document for review with a memo.  The idea is the person reviews the attached document, then, signs the pdf document it is attached to for approval.  I have sent this multiple times but it seems when i set up the adobe for signature it does not forward the attachment with the document.  Is this not possible?  Thanks
