auto-fill forms from a questionnaire - Best Practice

We want to present a questionnaire to applicants to fill out.  The questionnaire populates fields in a group of forms (Binder), gives the applicant the opportunity to review the forms and then prints them.  When the forms are printed we want all data to be erased in the questionnaire and the form.  Questions:  1.  Is there a way to do this entirely in Adobe Acrobat DC Pro?   2.  If not, what additional tools do you recommend, preferably Adobe.  3.  Can you outline how we would achieve this using the recommended tools?

Yes, this is easily done in Acrobat. Go to Tools - JavaScript - Set Document Actions and edit the Document Did Print command. Enter into it the following code:

 

this.resetForm();

 

That's all there is to it.

However, I would strongly recommend you inform your users of this "feature", or you'll get a lot of angry feedback down the line...
