I have been searching for clear instruction on how to create a fillable PDF form that allows the following:
A candidate needs to collect the following to be nominated:
- name
- address
- email
- phone
- e-signature
We need to allow a candidate to download the form and have at least 25 individuals complete the required information.
What is the best method to do this? I have Acrobat Pro DC. Thanks.
