changes to document not saving

Hi, I'm running the accessibility checker in Adobe Acrobat DC Pro.

Tools > Action Wizard > Make Accessible. Then following the prompts.

It tells me that table headers have failed.

So I right click on the first element and choose "show in tags panel" and fix the errors.

Then I go back to the accessibility checker and ask it to check again. The error is removed.

I save the file and reopen it and run the accessibility checker again.

The same error returns.

Please help.

Should have mention i'm running version 2019.010.20099. The file is stored locally, but i'm not the author, and I dont have access to the source document. Which I beleive would have originally been an MS Word doc.
