Create layer hierarchy (sublayers) in PDF

I have seen some PDF documents with a layer hierarchy: a layer (folder) with sublayers. The view of each (sub-)layer can turned on and off with the 'eye-button'. We have tried different things with the adobe programs (Illustrator, Photoshop, Indesign, Acrobat pro), but we still don't know how to make layerfolders and sublayers in pdf (which can turned on and off with the 'eye-button'). In the attachment you can find an example of this.

Can someone tell us how to create this in PDF?

It can't be done within Acrobat itself, but it will display it if it was created like that in other applications.

With Acrobat Pro it is easy to do the opposite, i.e. to put all layers at the same level.
But it does not allow to group them.
