Creating Custom Dynamic Stamps with Blank Text Fields

How can I create a custom dynamic stamp that keeps blank text fields so that it can be used in multiple documents?

You can't. Once a stamp was applied it becomes flattened. However, you can use a script to display a dialog window that will populate those fields when the stamp is applied with the values the user enters. But once applied it can only be moved, resized or deleted and replaced with a new stamp, not edited directly.

What do you mean by "multiple documents"? changed after it is applied? or just that you can apply the stamp many times, and fill in different values each time?
