edited / added to a PDF but text added text displays on multiple pages besides where I inputted text

Adobe acrobat DC - contributed to a fillable form but text inputted displays on multiple pages.  Please let me know where I went wrong.  Thanks!

Use unique names for the fields.

I ended up retyping all input from the original.  I should have stored in SharePoint for all to key in info on the original doc.
