Enforce Multiple Checkboxes Checked prior to Signing

I have a PDF form, (Statement of Understanding) with multiple lines the client will check and then sign. 

I would like to make sure all checkboxes are checked prior to signing. How do I enforce all check boxes are checked before the client can sign?

I would add a button that uses a custom script to verify all the check boxes are checked and if true then displays or unlocks the signature field.
