Fillable dynamic stamp

What is the javascript I need to use to make a Paid stamp fillable?  I want to add a date but not necessarily today's date.  I usually put in my initials and the date the bill was paid but usually the date is a previous date.  I had this before but our IT person had to redo my computer so EVERYTHING is lost!!!  I don't have the information on the javascript I used before. Please help!!

Take a look at this old tutorial from Thom Parker, who literally wrote the book on dynamic stamps:

Even though this was written for an older version of Acrobat, it still works the same way.
