Thanks, it was a PAC error, cant seem to replicate it. But the form has fields and form tags, but the PAC test says form tagsare not there, want to put second tags in some fields.

Hi, I know how to make form tags (annotations). I tag, put in form fields, find links then annotations. I place the form tags and links in the correct spots. Then I run PAC test and it says annotations missing. But they are not missing. For page 2, it makes 2 form tags for most fields. https://drive.google.com/open?id=1WifXM6X3UKjizZY6Kf8nkBm0gznwdJ0b

What version of PAC are you using? PAC 3 is the current version.

So in the link that Bevi Chagnon sent you there is another link with guidance for "The Basics of Remediating an Inaccessible PDF" : https://adobe.lookbookhq.com/acrobataccessibility/the-basics-of-remedi which in return led me to this other video https://taggedpdf.com/508-pdf-help-center/tab-order-entry-in-page-with-annotations-not-set-to-s/ This last link applies to PAC2 and it resolves another error that seems to be common but I think it may be useful to your case. With your document open make sure you have thumbnails preview opened and skip the video to the 2:26 part The author shows how to right-click on the selected thumbnailed pages to bring up the "Page Properties", then select "Tab Order Tab" and tick "Use Document Structure"... maybe in your PDF document you have this set to something different which may be why some of the tags that you've identified seem to be skipped in certain pages.
