Form Fields on a Layer

I'm trying to create a layer that once appears has form fields (such as checkboxes, text fields etc) that I can then fill out.  Is this possible and could someone steer me in the direction of how to go about doing it?

You could set the form fields to Hidden in the field properties dialog box. Then a button or other action could show all the hidden objects.
