Form - When i fill out my form the text is duplicated into every text box? I need to change this?

Form - When i go to fill the form in the allocated text box areas, the text is duplicated to every text field on the form, not sure how i can make it so you can write something different in every text box?

How do I rename the field's

They were auto grenerated by adobe
