How do I create a pdf document with functionality that only shows the viewer specific pages based on choices they make from check boxes on its contents page?

I usually use MS Powerpoint to create the document, then make the pdf from there. However, I'm uncertain of what additional software I might have to use to create the functionality required.

Any help or suggestions much appreciated!

You should use a layered PDF since it's easy to show/hide layers (aka OCGs).

This can be authored with InDesign, Scribus, etc., or directly with Acrobat Pro.
