No, this is the kind of thing that should be done before creating the PDF. It is possible, however, to do it using a script.

 

For example, I've created a (paid-for) tool that can create a TOC with clickable links and insert it into the file based on the bookmarks that already exist in it. You can find it here: http://try67.blogspot.com/2011/03/acrobat-create-toc-from-bookmarks.html
