How to send email using javaScript with a .pdf required attachment

I 've a PDF file with Five Files Attached. It has few fields to be filled also. One such field is Email ID.

When he clicks Yes to a certain column, then a button comes up to send documents. Here I need few of the files from the attached file lists to be attached (Should also be predefined) and the TO mail field to pre loaded from the above filled Email ID column.

Love to speak to some adobe experts.
