I am using 2015 Classic Version of Adobe Acrobat Pro DC and do not have the image field option under the Prepare toolbar. Is this not available on the classic 2015 version? If not, is there another user friendly way that I can add an image field so users can import an image into the completed pdf form?

I believe this tool was added to a later version of Acrobat than the one you have, but an image field is really just a button field with the Layout option set to Icon Only and a simple script on the Mouse Up event, which you can easily do yourself. The code to use is:

event.target.buttonImportIcon();

Have you created a Adobe Sign form? What can you see at the toolbar?
