Image not appearing in generated form [Forms Central]

I'm using Adobe Forms Central to create a fillable form. The graphic that I have placed in the header doesn't appear in the generated PDF. Why?

It may be possible that the PDF has enforced a PDF/A viewing standard when it was created and deleted that image to protect against reproduction of the embedded content.

You can disable this  before creating a PDF and try creating a document without enforcing PDF/A. 

Alternatively ,  you can enable/disable PDF/A viewing mode in the Preferences of your Acrobat.

Forms Central has been discontinued several years ago. Are you still using the app version of it?
