I have seen this issue on a lot of forums but none of the solutions i have seen seem to be working.
I have a PDF form set up. Text fields print fine but when a check box is checked the check mark does not print.
I am creating on a Mac using Acrobat Pro DC 2015 release.
Any idea why?

I have the same problem with Adobe Acrobat pro XI. But I did a workaround by superimposing a text field on top of the checkbox field. This is fine, but tedious.
Then after playing around, I found that Acrobat Pro 9 did not have this issue. So I uninstalled XI and installed 9 in its place. I hope somebody has a workaround for this problem with XI.
What I also noticed is that if you bring up the filled-in form, manually erase and re"check" the checkbox, it prints fine.
Then I also noticed that the checkbox field properties use "Adobe PI" font, which is hard-coded and cannot be changed. So figuring that perhaps the font was missing, I copied the AdobePI font to Adobe Acrobat "Fonts" folder as well as the folder in which the form document is residing. No luck.
Any help would be appreciated. Right now I have solved the problem with XI by replacing it with Acrobat 9.
