Pdf form duplicated info

I am new to pdf forms so forgive me if this is a silly question. I have a 12 page form where each page is filled with fields. I copied and pasted the format fields to speed up the formatting process but now that I am testing the form, when I enter info into one field, that info is now copied throughout the page. How do I change it so each field will carry it's own unique info or should I redo the form from scratch and insert fillable fields for each individual area?

Use unique names for fields.
