Has anybody ever experienced the situation where a document which has been redacted (using Acrobat DC) has had the redactions "undone"?

You need to understand the difference between creating Redaction annotations and applying them. Redaction annotations can be easily removed and then the contents underneath them is revealed (even if they are not removed that content can be accessed). Once they are applied, though, the data is completely removed from the file and can't be restored.

As per the issue description mentioned above, you want to know if redaction in Acrobat DC can be reversed, is that correct?
Sorry to say but once redaction has been done on the pdf, it can't be reversed. Refer to the following forum threads discussing the similar topic:
