Signature fields not showing up?

When I send a PDF for signatures--to my other email address--the placed fields are not there to sign. Where would they go?  There is no interaction to "save" those fields before sending them. 

Any ideas?

Did you use Adobe Sign with this PDF?

I was in Acrobat and Requested Signatures from the Sign menu. Yes, it looks like it is powered by Adobe Sign.
