Tagged TOC Items Not Read by NVDA Screenreader

TOC items in our manual are tagged as follows but when it is being read by NVDA screenreader, the TOC item are read as <name of product> document%G4... link when it should be <TOC item name><page number>:

How do i address this issue? The manual was developed using Framemaker 2015 and was Tagged feature enabled and was further updated for accessibility using Acrobat Pro XI. Note that we have recently upgraded to Framemaker 2020 and Acrobat Pro DC
