Can Adobe create a survey form for my business?

I have an existing form that is an extensive survey tool in an unmanageable format.  Can Adobe provide services to streamline completion of survey forms?

Adobe doesn't provide form-creating services, but we do have several applications that allow you to create your own forms. You can use either Acrobat or Adobe LiveCycle.
