Hi jashmos2,

What version of Acrobat Pro do you have? You can do this using LiveCycle Designer, which is included with Acrobat X, but not with the current version Acrobat XI.

Another option is to create a button that prompts the user to select an image. Please see this thread for details: Insert editable picture into PDF form?

We had Adobe Acrobat X, but upgraded to XI. I called Adobe support and they told me that Adobe LiveCycle Desiger costs $239. Seems a bit much $, but does LiveCycle do everything that  Adobe Acrobat does?
