Combing Files Duplicating pages

I'm attempting to combine multiple pdf files into one pdf file.  I have 3 pages that are based off the same format with input feilds (simple text strings).  Each time I combine it duplicates random feilds from page 2 to page 3 and links them together.  Thus they have the same text and if i change on one page it makes the SAME change to the other page.  I have combined many documents using these same templates and have not run into this issue.  I have combined online and on my desk top with the same results. 

If I combine as a Portfolio there is no duplication, but I would rather not use the Portfolio format for these reports.

Any ideas???

Change the names of the form fields or flatten the form before you combine the files.
