convert edited .pdf to word, but edited text fields to converting

HI there,

I have created a fillable form in .pdf. Clients are required to fill in the required fields which I then want to convert to word for my use (my questions and their answers), but for some reason only the form is converted and not the fields they entered. That is blank. How can I fix this please.
