Covert pdf to excel and/ word

I have a pdf customer order form in pdf that I wish to convert to Word and/or Excel

Hi Minakshi

Thanks. I was able to convert to Excel . However some merged cells appear
distorted. Attached is the pdf, Excel distorted and my attempt to correct.
I would appreciate if you could take a look and advise.
