How can I get others to fill and sign a form I create?

I'm creating a PDF file, where others can fill in either answers on lines or check boxes. What do I need to do that?

If you have Acrobat, you can create a form from your PDF using that. If not, you could use the Adobe FormsCentral online service to create a form. Do you have either of those? See www.adobe.com/products/acrobat.html or http://formscentral.acrobat.com.
