how do I submit a form from my website

Preface this by saying I am 64 years young and painstakingly designed my biz website using FrontPage years ago. I do not want to have it redesigned by some web developer--don't have the $$ or energy to do so. I can ftp all my pages just peachy, but GoDaddy no longer supports FP extensions, so the only form I use for clients to fill in and sign up for services NO LONGER WORKS! I have spent over a week, diligently working/attempting to convert my form to something usable on my site...but can't. I have redesigned the form in OmniForm (which I'm familiar with) and then printed to PDF, and it looks perfect. However, users will have to figure out how to enter info, then spend time and cross their fingers trying to email it to me. Are tab stops available? When I try to plunk the form into my site, of course, I need to do far more than I know how. Presently, I would have to send the pdf, along with detailed instructions, to prospective clients for them to fill out and email back to me. Far too much work, and I wouldn't want to muck with the process. Can somebody help??
