PDF Form displays "Flash" in tab instead of file name

I used Acrobat Pro to add Form fields to an existing PDF. Now, whenever I open the file the tab says "Flash" instead of showing the file's name. Even the taskbar shows the file's name as Flash!

I'm running Windows 11 and have been using Acrobat for decades, and have created many forms.

How can I get the file name to display?
