pdf to docx for square check boxes on application form.

I am converting a pdf to docx and the pdf has squares for checking the box in the pdf. But when converted it shows up like a little house or saw blade. How do I get it to keep the simple square as in the original pdf?

Could you let us know if you are converting pdf using export pdf option available online?
