Adobe Sign Not Returning Form

I sent out forms to be signed and initialed  which was completed by the signee.  The signee was returned a completion audit but I the sender was not and it is still in my file box as not completed.  When I open the file - It shows all fields completed. What is going on with this?

- this happened once before about 3 months ago
