Calculation from Radio Button not transferring in Adobe Sign

I am creating a form in Acrobat DC and will then convert to an Adobe Sign form. I used JavaScript (in custom calculation script) to add a number in a field based on which radio button is selected. When the form is converted to Adobe Sign and sent out for signatures, the JavaScript does not transfer; the form is flattened and calculations are not transferred. I read that adding the calculation as a text tag in the field name works but I can't figure it out for this form. I have five groups of radio buttons, each is assigned a number 1 - 5, so when I click button 1, a 1 is populated in another field. I then use those fields to calculate an average. I am not sure how to define the field and use if statements as a Text Tag. Or is there some other way? This form is for performance evaluations, so the employee needs to be able to enter their fields then the manager, and both sign.
