I have checked boxes in filled forms that I need to get signatures for, but everytime I click request signatures and prepare the document, it either unchecks the boxes or inserts new checkboxes. It does not remove already filled in text nor radio boxes. I've tried this on the website, adobe.acrobat.com too and that is not a solution because it routinely crashes or cannot finish creating the documents. Any advice on how I can fix this issue in the desktop app?

In the meantime, we suggest using the workaround. Add the Checkboxes in Acrobat Sign.

I'm now experiencing this problem in a different form on the new Adobe Sign UI on the website version. I have a pre-filled out form with several checkboxes marked, and in some signed versions, either some checkboxes are unchecked, or some rows of checkboxes (same name different export value) are all checked, when only one was prior to completion.

If the form fields have the same name, the information filled in one field will automatically be reflected in the others. It is the designed behavior of forms.
