I can not turn off the auto populate signatures and dates, please help

Greetings, I have business documents that need to be signed by clients multiple times a day.  They appear to have auto populate signature and dates that populate when I create the pdf for signature when I'm in the "Request Signature" link.  Prior to entering the "Request Signature" link, I "fill" the pdf with the dates that are required.  Unfortunately, these dates that I just filled in, are then written over (blanked out) by the auto populated dates.  This is also happening in the signature fields that I "fill" with my own signature prior to entering the "Request Signature" link.  Needless to say this is a big pain and I believe it has just started happening recently...within the past couple months I believe.  The Advance mode turned off seems to work the best but unfortunately I do not have an option to turn it off when I use the original pdf form.  The "fix" I received from CS was to create a "new" pdf and then apply the signatures.  This has been an extra step but now it is not even an option on the new computer I just bought.  Is there a way to turn off these auto populating signature and date fields?  Thanks in advance!
