Prepare form - Add signature / initails option missing?

I have prepared a document with all sorts of fields, and I am at the end of this process, and adding a signature and initial fields options are not available. I have made numerous other documents without a problem, so this is really confusing me.

To add the signature fields, make sure you first convert the form to an Adobe Sign form from the More menu
