I created an employee evaluation form using Web Forms. I used radio groups with assigned values of 1-5 for the different areas that are reviewed. There are a total of 6 areas. All radio groups are set as required. The form then uses a formula to calculate the average rating of the evaluation.
When my staff member fill out the form, the finalized agreement only shows that they made a selection on 4 out of the 6 groups (although all 6 groups are set as required and the staff member confirmed they made a selection for all categories).
I submitted a test form and got the same results - the form only shows that I filled out 3 of the 6 required categories.
The average rating appears correct on my form. My staff member claims they made different selections on their form than what is shown on the final agreement, but I cannot verify that.
Can anyone think of why the chosen radio button is not shown on the final agreement?

Please let us know how you have added those radio buttons. Are you adding buttons via the Acrobat desktop application or in Acrobat Sign?
If you can share the screen recording of how the PDF form is filled and the results you receive, you can send the form to your alternate email address to test. If it is a web form, then you will be able to use it easily for testing.
It seems there is an issue with how the form fields are added, which might be causing the issue. Please refer to the information in the following help document: https://adobe.ly/4icGL1k.
We will require more information to look into this.

Attached is a copy of the final form. All of the selected radio buttons on the final agreement have been shifted to the right - meaning that if I selected the first radio button in a group of five, on the final agreement it shows that I selected the second radio button.
