Recipient Signatures ignored during webform

I've created a web form for a specific agreement. The first recipient fills in the necessary information and then it is passed along to the next recipients for their signature. Upon testing, those signature fields are ignored and recipients are forced to sign on a separate page from the agreement, instead of the desired signature fields. What would be the reason behind this and how would I go about fixing this? I'm on an enterprise account with admin access. It seems to be issue related to the form filled not assigned. Please ensure that you have assigned the form to the respective signers. 

If that is not the case, would you mind sharing the screen recording of the workflow you use to send that document? You may try sending it to your alternate email addresses for testing. It will give us a better understanding of what is happening.

So I actually did not change much except for removing the name and initial field and it now works. Not really sure why, but I'll accept it. Maybe posting visual references of what your template looks like could help?
