Removing extra page in Widget

When Creating a widget, there is always an extra page added to the end of my document. As you can see in the photo, it's just a blank sheet asking for fields that are already in my form.

I contacted support and they said that an email and signature block need to be in the document...both fields are present in my document already.

Is it at all possible to remove this extra page?

Hello Zach,

By default, Widgets required to have an email field. Can you check if you have placed Email address field in it or not? Also, it seems that you have set Title field required in Signature Preferences. You can go to Account tab->Account setting->Signature Preferences and under Additional settings, uncheck "Requires signer to provide Job Title".
