Salesforce Integration - Add Checkbox in Form

Hello together,

 

I just implemented Adobe Docu Sign in our Salesforce environment. In our contract templates there is a checkbox our customer can set as an optional value. We do not have a direct connection to a field in Salesforce.

How can I implement a checkbox field to an agreement template (Word template)? Unfortunately I couldn't find anything in the Adobe documentation.
