Something went wrong after client filled out the forms

So my client has to fill out the form 3 times but it always says something went wrong as seen in the screenshot. It will redirect and client will fill it again, and then it would do the same thing again. How to fix this? The prompt is seen in the screenshot.

In case you are still experiencing the issue, we see that the user is opening the document via the mobile. Please ask the user to try it on the desktop once.

Try clearing the cache and cookies for the browser on the mobile device. Check if that helps. If the user still experiences the issue, let us know.
