Using Adobe Sign for Dynamics 365 and defining fields for the signer to fill in other than signature

When we use the Adobe Sign Templates with the Adobe Sign plugin for Dynamics 365 we can get the defined fields to be included in the required fields for all the participants.  However, we want to be able to do the same thing with our own PDF documents sotred locally.  We need to use the Data Mapping, as well as require the participants to fill in the rest of the fields before signing.  We have tried to download the PDF from the Adobe Sign Template, but that ignores the fields when sent out for signing.
