Adding Form Fields Option Not Available

Please advise.  I just signed up for Isign and I am not seeing the check box or click button option to add form fields to my document.  I have combed through the help and followed all the tutorials...the option isn't there.  Please help.

Is this happening with all the files or just the one file?
