Standard form; initiated by user with 2nd signer.

I'm trying to create a standard timesheet-style form for use by student-interns who need to provide all the information about what they worked on (including dates, hours, description) over a 2-week period, then forward this to their mentor for signature, before returning it to the correct teacher. Creating the form for two signers was fairly easy. Now, how can we make it available to students who need to access a new copy every two weeks and allow them to forward it to their mentor for approval before returning it to their teacher without the teacher needing to be involved in every step. This program involves well more than 100 students so we need an efficient fix and it needs to be secure so that we know the mentor is the one approving the time.  Thanks!

I would create a form in Adobe Sign, and place the teacher as a co-signer, give the link to the students.

If it is not the same teacher, you would have to create a form for each teacher.
