Auto Orient not working: I need to make a layer face the camera all the time.
Does this setting require something other than rotation?

I found the culprit: The after effects orient to camera doesn't work correctly orthogonal. I don't know what it actually does but I do know it is not the result one would expect.

Just parent the layer Y rotation to the camera Y rotation. Much better!

Only the Active Camera will render. If you want to get close to an orthographic view, you'll need to add a camera with a very long lens. The easiest way to create something close to an orthographic view is to set the viewing angle to about 1 or 2º. None of the other views will render. They will preview, but they will not render.
