MP4 files can be easily corrupted and often cause problems. The solution is transcoding them to a visually lossless production format like ProRez. You can use the Adobe Media Encoder or maybe even Premiere Pro to do that.

You are trying to add metadata and it simply cannot be added. Why and how is impossible to figure out just by throwing the render report at us. You'd need to provide much more info.
