I have what may be a related issue. When first opening a folder of say 800 images, I select all of them and it shows that all 800 are selected at the bottom left. Bridge is still building previews using the "embedded" option. When I run the rename preset (run many times before) not all files get renamed, missing a handful. When I run the same preset on the remaining group, the files are renamed, but the timestamp is not complete, missing the milliseconds. File info shows the milliseconds.

When you first open the folder and go to rename, you should wait until Bridge is done thinking about the images in that folder before selecting all and starting the renaming process. You can tell when it is still thinking about the images by looking at the activity monitor in the lower left. If it is still generating thumbnail previews and what-not, they may not be selecting correctly, hence the missed files. And having 800 images in one folder can take it a moment to organize, correlate and prepare all of those images, especially if they are larger RAW files. Just give it a moment first, then rename.
