Tools > Photoshop > Contact Sheet II

Will mostly do what you want, however there is only a filename option and no metadata option.

Otherwise a custom Bridge script or perhaps a contact sheet script for InDesign would offer more flexibility…

The raw data can be exported out of Bridge, however it takes work to use it either in other Adobe apps, a webpage etc.
