Am I correct in thinking that we have to manually apply denoise in camera raw for each image file? We can't include that setting in sync?

You should be able to sync your Denoise edits to multiple photos in Camera Raw. How are you currently copying your edits to other photos? In this example, I selected multiple images in the filmstrip, then clicked on the Synchronize button to get this menu. Make sure that Detail > Denoise is checked.
When you apply Denoise, the resulting file will be saved as a new DNG, with the file name appended with “-Enhanced-NR”.

Oh, oops, I didn't see that, thanks. And yeah, the filename addition of “-Enhanced-NR” is super annoying.
