Can't seem to figure out why the answers (after answer "B") are misaligned from the first two options. I've even remade the slide from scratch and it still continues to be an issue. How do I fix this alignment issue?

Can you tell which Theme you are using? And which version (as mentioned under Help, About Captivate)?

I'm using Adobe Captivate 2019 v 11.5.5.676
I believe I'm using the theme "Alliance".

Alliance is one of the Quick Start Projects. It is near midnight here, will have a look at the quizzing master slides tomorrow in that Theme.
However you are on Mac, I am on Windows. That may be a problem.

I've seen this problem before and one solution is to reset the Object Style on each of these answer objects.
I'm suggesting this because the first two answers are aligned differently to the rest. That's the give away because when you insert a quiz question into Captivate, by default it only has two answers. You add the rest from the Quiz tab.
So what I think has happened here is that the first two answers have the default Object Style and the rest have a slightly altered Object Style. You want them to all look the same. So, by selecting each item and then resetting their Object Style in the Properties tab you may be able to get them to all have the same alignment.

The reason I asked for more time is that the Quick Start Projects can be tricky if you use indeed their theme in your project. If you are sure that you use the Alliance theme for this quiz slide, the (partial) screenshot looks different from the quizzing master slide for MCQ.
I double-checked its behavior and couldn't reproduce the problem you have. Rod suggested to check the Question Answer Style. In Alliance it is using Tahoma 18pt, left aligned and vertically centered.
Will not explain more, but it looks like you have edited the styles of the quiz slide. Did you do that on the master slide or on individual quiz slides?
Some QSPs have a theme which is not very well designed for quiz slides, but that is not the case for Alliance. I summarized the possible issues:
I couldn't detect if you are on a responsive or a non-responsive project from the question. There is a similar post for responsive projects.
